careers. Everybody knows that the fewer steps one has to take to complete a
task makes a job easier and faster.
With that said I now will introduce you to one of these time savers--Online Forms.
A form is a structured document with spaces reserved for entering information
and collecting data.
Not to long ago, forms were printed out then physically mailed. Today, though,
with new technologies and the World Wide Web available, Online forms are the
way to go.
In this tutorial, Word 2000 will be used to create an Online Questionnaire Form.
So, take a deep breath and relax for this really is a simple process.
It is always an excellent idea to do an initial layout of your form on paper.
The reason for doing this are so you can decide

The name of your form
-
Which clip art or pictures you will use
-
What questions will be asked
-
The placement of these items
Advance planning will minimize your overall time spent, so do your rough
designing on paper instead of on a computer.
To create an online form, first you must create a template.
A template determines the basic structure for a document and
contains document settings such as fonts, page layout, formatting, styles,
lines spacing and so on.
The reason for saving the form as a template is that if you saved the form as a
document. the users will open it and then fill it out then they have to save
the form with a new name to preserve the content of the original form. They may
not even bother to, and that isn’t good.
However, if you create your form as a template, the original form is preserved.
When they open the form, it is presented to them as a new document based on the
original form’s template. Then all they have to do is fill in the form click
the save button and email it back to you.
You will need to perform the following steps to start Word, create a template,
and then save the template as Jamestown Motocross Questionnaire.
To start word:

Click the start button on the task bar

Click New Office Document on the start menu
-
Click the General Tab when the dialog box displays
New Office Dialog display box:

Double click the blank document icon
-
Maximize your word window. (You can do this by clicking the middle button on
the right side of your title bar, or by double clicking the title bar itself.)

Click the zoom box arrow on the standard toolbar, and click page width.

Turn your formatting marks on.

Yes, I know you are past this stage, but it will help you later on. Trust me.
Creating Templates.

With your Word window open click Fileon the menu bar ,then click New.
-
Click the blank document icon under the general tab. In the create new
selection, click template. Then click ok.

Click the Save button on Standard tool bar.

In the file name text box type “Jamestown Moto-X- Questionnaire”
-
Change the Save in box to My Docs, click the new folder icon name the folder
Form Tut.

Double click the new folder to open it then click the save button.
(You will notice that the Save as type box is grayed out. This means you cannot
change the document type. Nor do you want to.)

Other ways to bring up the Save As dialog box:
File/Save As
CTRL + S
Enter and Modify Text

Click the Font box arrow, scroll down and select Baskerville Old Face

Click the Font size arrow box and select 36.

Click the bold button, then click the center button
-
Click the Font color button and select Dark Blue
-
Type “Jamestown Moto –X- Park” then press the enter key twice.

Click the Font box arrow; scroll down and select Arial
-
Click the Font size arrow box and select 28
-
Type “Pro Riders” press the enter key, and then type “Questionnaire” Press the
enter key again
